Issues with mediawiki editor

Description

On MIKE2.0, when you go to edit a page, there is a "[Disable rich editor]" setting that appears above the page. Need to make this standard across omCollab installs.

Also, open up a page, e.g.  http://mike2.openmethodology.org/w/index.php?title=MIKE2:Content_Model&action=edit The page shows up below the page in the fckeditor and why is width of the edit page small

We should have the following:

  1. rich editor does not come up by default
  2. users can toggle on rich editor as on current site
  3. width of page in rich editor is the same as without rich editor
  4. no extra edit page when in rich editor below the wysiwyg page

Issues appear differently with different browers. Kevin replicated the problem with: The urls below in windows 7 and ubuntu 9.10 , GC 4, 5 and FF 3.60
